Most of our faculty and staff have been vaccinated against COVID-19 and we expect our students to be vaccinated as well. We must remember that there are members of our community who qualify for vaccine exemption and our youngest students may still be awaiting a vaccine. Depending upon the need, we may have to put policies and protocols in place at a later date.  

If your student has received a vaccine, please send a copy of their vaccination card to Melissa Kruse, mkruse@berkshirecountryday.org.

• Physical Examination and Immunization Record – Massachusetts  requires that the school have a current yearly physical on file. The school accepts physicals and immunizations on various forms from the doctor’s office, you do not need to send both forms.
• Prescription Medication Form – is required if your child will receive prescription medication daily during school.
• Asthma or Allergy Form – is required if student(s) will receive an EPI-pen or are on a Management Plan.
